Admission Procedure

EMERGENCY ADMISSION

Patients and their companions will be guided to the Emergency Department (ED) at G/F for a comprehensive medical evaluation. If requested by the ED admitting physician, they will be directed to the Admitting Office to facilitate the admission process.

NON-EMERGENCY ADMISSIONS

For admission, patients and their companions are advised to visit the Admitting Section at G/F.

Reminders:

  1. The room will be assigned on the day of admission.
  2. Please call the Admitting Office three (3) hours before the actual admission.
Admission Procedures:
  1. On the day of admission, proceed to the Admitting Section and get a queuing number at the Admission Area.
  2. Get Patient Information Sheet and fill it out completely and accurately.
  3. Stay at the Admitting Waiting Area and wait for your number to be called.
  4. Proceed to the assigned Admitting Counter once your number is called or flashed on the queue screen.
  5. Present the following documents to the Admitting Staff:
    • Physician’s Admitting Order Sheet
    • Company ID/HMO Card (for Company/HMO Charge Account)
    • Any valid Identification Card (ID) with home address (e.g., Company ID; Driver’s License; Voter’s ID; Passport; SSS; BIR/ TIN ID)
    • Birth Certificate for underage children and guardians valid ID.
    • PhilHealth Claim Form – Claim Signature Form (CSF) (if applicable).
  6. Also present:
    • Identification Card (i.e., HMO, Company)
    • Discount Cards (i.e., Senior Citizen Discount, PWD)
    • Letter of Authorization (LOA) (for HMO, Company and Stockholder Account Only)
  7. Listen carefully to the explanation of Admitting Staff regarding admission guidelines and ask for further clarification
  8. Acknowledge the following items from the Admitting Staff
    • Duplicate copy of Admission Checklist
    • Duplicate copy of Consent Form for Admission
    • Acknowledge receipt of Television/Cable Remote Form
    • Admitting kit
    • Vacuum Pillow
  9. Read the Patient’s Rights and Responsibilities while waiting for the processing of admission. If you have any clarifications, you may approach the Admitting Staff-in-charge.
  10. Sign the Patient Data Sheet and Consent for Admission and Treatment
  11. For Personal Charge Account, you may approach the Cash Management Department for further instructions.
  12. Proceed to the Admitting Waiting Area and wait for the Transport Orderly who will accompany you to the room.
What to bring on admission day:
  1. Required documents/Identification Cards/discount cards such as
    • Letter of Authorization (LOA) (for HMO/Company Charge Account/International insurance)
    • Physicians Admitting Order Sheet
    • Results of Diagnostics Procedures
    • Company ID/HMO Card (for Company/HMO Charge Account)
    • Discount Cards (i.e., Senior Citizen Discount/PWD)
    • Any valid Identification Card (ID) with home address (e.g., Company ID; Driver’s License; Voter’s ID; Passport; SSS; BIR/ TIN ID)
  2. You will only need to bring a few simple things for your hospital stay. These might include toiletries, a robe and non-skid slippers, nightclothes, and reading material. Hospital gowns will be provided if you choose to wear one.
What NOT to bring on admission day:
  • Do not bring any medications except for chemotherapy/anti-neoplastic medicines/maintenance medication.
  • Do not bring any valuables, jewelry or large amounts of money. Hospital will not be liable for any lost.
  • Do not bring any personal electrical appliances (e.g., water heater, hair dryer, flat iron) and furniture (e.g., foam, mats, chair, folding bed)
  • Do not bring any alcoholic beverages.

NOTE: FOR SCHEDULED ADMISSION PATIENTS MAY REQUEST TO BE LINED UP IN ADVANCE. THE REQUESTED ROOM CAN ONLY BE CONFIRMED ON THE DAY OF ADMISSION.

Guidelines:
  • Visitors should wear facemask and without respiratory symptoms.
  • The visitor(s) must present a valid id in exchange for a visitor's pass.
  • Two (2) visitors at a time during visiting hours to avoid overcrowding.
  • 7 years old and above are allowed however, ages from 7 years old to 18 years old should sign a waiver indicating that Julius K. Quiambao Medical and Wellness Center is not liable for any disease or infection that may acquire.
PATIENT DATA SHEET

The patient is required to thoroughly complete the Patient Data Sheet, providing all necessary information such as personal details, medical history, and contact information, to ensure accurate, comprehensive, and up-to-date records for effective care and communication
