Pwede nating i-check sa Philhealth Section o sa Admitting Section kung tayo ay Eligible gumamit ng Philhealth. Para macheck ibigay lamang ang detalye'ng kailangan:
- Buong pangalan ng member, address, birthday, at contact number.
- Kung dependent naman tayo ng member (asawa o anak below 21 years old) buong pangalan ng DEPENDENT at birthday.
- Kung tayo ay “Eligible – YES” wala nang ibang documents ang hihingin sa atin.
- Kung tayo ay “Eligible – No” ang mga sumusunod ay kailangan nating maiprovide.
MGA DOKUMENTONG KAILANGAN
- CSF (fully accomplished Philhealth CSF Form)
- MDR (Member Data Record)
- Proof of 9 months premium contribution within the last 12 months prior to the 1st day of confinement
- Proof of 9 months premium contribution within the last 12 month prior to the 1st day of confinement or payments amounting to P 2,400.00
- CE-1 (Certificate of Eligibility issued by Philhealth)
- Philhealth Lifetime Member ID Card / Kung hindi pa nakaregister as Philhealth Member maaring pumunta sa pinakamalapit na Philhealth Office para mai-register (PMRF with OSCA ID)
- Proof of premium contribution for the whole year (P2,400.00) that indicates “Women about to give Birth”
Iba pang detalye:
Kung sakaling hindi nakalista sa MDR and qualified dependent, magsubmit lamang ng proof of relationship sa Philhealth Officer ng ospital:
- Legal na ASAWA – Marriage Contract
- ANAK na 20 anyos pababa (walang asawa at walang trabaho) – Birth Certificate
- ANAK na 21 anyos pataas na may angking kapansanan (mental/pisikal) – Birth Certificate at Medical Certificate
